Population Explorer
Population Explorer is a valuable tool within the WVHIN portal that lets healthcare providers review detailed patient encounter histories without any additional login. Robust clinical information, referrals, and controlled substance monitoring data can be filtered or summarized into a snapshot. CSS* Event Notification Delivery (CEND) enables real-time alerts that can be customized to monitor patient admissions and critical health changes, accelerating care.

SDOH
Under the Social Needs tab of the WVHIN portal, SDOH screening assessments provide details about social determinants of health, including food insecurity, housing instability, and financial demographics. Sourced from ADT feeds related to ICD-10 codes, this data supports mandatory and voluntary SDOH reporting to assist with Medicare and Medicaid reimbursements. These assessments can help analyze community needs, inform care decisions, and personalize benefits for members.
Consent Tool
WVHIN’s consent management solution enables the secure exchange of substance use disorder (SUD) data protected under 42 CFR Part 2. Electronic signatures let patients consent to share confidential SUD data with their care team to optimize treatment, offering flexible expiration dates that give patients control over their information. Once providers register signed consent from a patient, their SUD data integrates seamlessly into existing clinical workflows.
Reports & Alerts
WVHIN allows participants to customize a variety of alerts, notifications, and demographic reports to receive real-time data and insights about their patients. Through CEND ADT notifications, smart alerts, updated demographic data, HL7 LOINC (Logical Observation Identifiers Names and Codes)-coded lab files, WVSIIS (West Virginia Statewide Immunization Information System) immunization data, and Continuity of Care (CCD) documents, users can receive tailored push notifications to enable timely interventions, enhance patient outreach, and close critical gaps in care.
