The WV Office of Epidemiology and Prevention Services (OEPS) in collaboration with the WV Health Information Network (WVHIN) is providing financial assistance to West Virginia Hospital Emergency Departments and Urgent Care Centers to achieve connectivity to the National Syndromic Surveillance Program (NSSP), also known as BioSense 2.0, through the WVHIN’s Health Information Exchange (HIE). Requirements for Grant Request

The following forms are required to be submitted:
  • Application
  • Detailed Line Item Budget
  • Detailed Line Item Budget Instructions
  • WVHIN Participation Agreement (Full Service)
  • Verification of current WV Secretary of State registration
  • Verification of active System for Award Management registration
  • Documentation to support the budget must be included. Written estimates, quotes, bids, etc. are acceptable documents.
